Here it is. A GRAYBAR 3 Blade fan, the name plate is dirty & hard to read but I think it says Type AOU, 28292 Form AF2, Catalog 75G425, 8PF0272189-1, 60 cycle, 110 volts. This is a heavy well built fan. The fan is approx 17" across the cage, and about 20" tall. The base is approx. 7 3/8" across, and weighs 19 lbs.
