It is from the National Life Insurance Co. OF THE U.S. of A. Super size the scan to read all the fine print. Measurements Card is 2 1/8" by 4 16/16" Condition Excellent Condition for age. Check picture for other details. These Cases are from my personal collections of stuff, which I am selling because I have run out of room. I will be listing other items soon, so check back.
